RAF1 — Claim Form Assistance
Completing the RAF1 lodgement form. Medical history, injury summary and supporting documentation included.

Treating Doctor Report
Comprehensive narrative report detailing injuries, treatment received, prognosis and functional limitations.

RAF4 — Serious Injury Assessment
Formal serious injury assessment using AMA Guides 6th Edition. Whole person impairment (WPI) rating included.

Narrative Test Report
Narrative test where WPI falls below 30%. Evaluates impact on daily activities, work and quality of life.

Short Report
Short-form medical reports for insurance, UIF and employer submissions.
